Thank you for your interest in an Innate Sleep Mattress. Our mattresses have been approved to be purchased with FSA and HSA accounts, but there are several steps that must be followed for you to qualify to use your FSA/HSA funds.
Step 1: Get Doctor Authorization
A letter of medical necessity (LMN) is required for reimbursement with a flexible spending account (FSA), health savings account (HSA) or a health reimbursement arrangement (HRA).
Your physician can provide you with this letter, or you can speak with one of our licensed medical professionals who will conduct a brief interview and determine if there is a medical necessity.
Step 1 (optional): Contact Our Office for Authorization
To schedule an appointment with one of our medical specialists, click the button and select a time that is convenient for you to speak with them. You will be asked various medical questions about your health, so you should plan on being in a place that provides the privacy to allow you to speak freely about your conditions.
Step 2: Submit Your Form
You will need to submit your authorization letter. Please take a photo with your phone, or if you have a scanner, scan the document and upload it here.

Step 3: Approval Confirmation and Ordering
Once your authorization form is approved, you will be able to order your bed. You have the option to purchase online or over the phone with one of our helpful team members.
